Partnership in Delivering Life Enrichment Survival Kits

This week we are partnering with Presbyterian Homes & Services to provide transportation and deliver Life Enrichment Survival Kits to ALL 48 PHS sites in MN, WI and Iowa to support resident engagement and person centered care.

The kits contain tools that will provide immediate resources for site level teams to:

• Make special connections with residents during additional one to one visits
• Provide extra resources for in-room independent activities that residents enjoy.

Presbyterian Homes & Services based in St. Paul, Minnesota, is a non-profit, faith-based organization providing a broad array of high quality housing choices, care and service options for older adults. 6,620 employees serve 27,000 older adults through 48 PHS-affiliated senior living communities in Minnesota, Wisconsin and Iowa, and through Optage® home and community services.
